On 19 November 2024, a letter of intent was signed by the Fédération des hôpitaux luxembourgeois (FHL), the Luxembourg government, and the European Commission setting out two objectives:
- Simplification of administrative procedures and exchanges between hospitals and the Joint Sickness Insurance Scheme (JSIS). To this end, a new systematic direct billing system (automatic direct billing) will be put in place for JSIS beneficiaries.
- Determining the costs of hospital services in a more targeted manner.
An administrative arrangement, signed on 25/09/2025, lays down the rules for collaboration between Luxembourg hospitals and the PMO, as regards administrative exchanges and the payment of care.

How does it work?
If you (or a member of your family covered by JSIS primary cover) must go to hospital for:
- Hospitalisation
- Childbirth
- Heavy outpatient treatment
Follow these simple steps:
Is it mandatory?
No. This is an optional service offered for your convenience. You can still choose to pay your hospital invoices yourself and submit a standard reimbursement request to JSIS.
Which hospitals and centres are concerned?
- HRS (Hôpitaux Robert Schuman): Centre hospitalier du Kirchberg, Clinique privée du Docteur Bohler, ZithaKlinik)
- CHL (Centre Hospitalier de Luxembourg)
- INCCI (National Institute for Cardiac Surgery and Interventional Cardiology)
- CHDN (Centre Hospitalier du Nord)
- CHEM (Centre Hospitalier Emile Mayrisch)
- Centre François Baclesse
Which treatments are automatically covered?
- Hospitalisation or childbirth, including care provided by surgeons, anaesthetists, and medical assistants.
- Pre-hospitalisation patient pathway (SPH).
- Major outpatient treatments, such as dialysis or chemotherapy.
- Injections at the ophthalmology outpatient clinic.
- Non-surgical day hospital care, or an invasive and/or high-cost examination
- Surgical day hospital care, such as a colonoscopy

Which treatments require prior authorisation for direct billing coverage?
Some treatments require prior authorisation.
If you haven’t taken the necessary steps to obtain it, JSIS may recover the full amount of the invoices if the services are found to be non-reimbursable.
It is therefore important to check in advance whether your procedure/treatment requires authorisation.

Which services are excluded from automatic direct billing?
- Any procedure related to orthodontics, periodontics, dental implants, or dental prostheses
- Medically assisted reproduction (PMA) — including insemination, fertilisation, and embryo reception
- Any surgical procedure related to gender reassignment
- Hospital stays in psychiatric care units (except in emergencies)
- Any aesthetic plastic surgery
For some of these treatments, this does not mean JSIS direct billing is not possible — but you will need to submit a standard prior authorisation request to JSIS, as your file must be reviewed in advance.

What are the other benefits of this change?
Alongside the move to automatic direct billing, the PMO is working with DIGIT to replace the current paper-based hospital invoicing system with a fully digital process.
